ABSTRACT

Objective To observe the efficacy of continuous renal replacement therapy (CRRT) combined with hemoperfusion (HP) on patients with severe acute aconitine poisoning. Methods One patient suffered from severe acute aconitine poisoning was admitted into the Department of Critical Care Medicine of Affiliated Hospital of Guizhou Medical University, besides the gastric lavage and multi-organ supportive conventional therapies, CRRT combined with HP was given to the patient and his clinical data of life vital signs and other parameters were observed. Results After the patient underwent 4 episodes of CRRT (102 hours in total) and HP (8 hours in total) based on the conventional treatment, 4 days later, the disease situations were gradually stabilized, all of their parameters became gradually normal, the patient successfully weaned off from mechanical ventilation, the via mouth tracheal intubation was pulled out and the patient was transferred out of department of critical care medicine. Conclusion In the patient with acute aconitine poisoning, it is necessary to use CRRT combined with HP as early as possible, that can effectively eliminate the toxicant in the body and quickly correct the electrolytic and acid-base disturbances, so as the above combine method can improve the patient's prognosis and elevate the successful rescue rate.

ABSTRACT

Objective To study the characteristics and significances of electrocardiogram(ECG)in the patients with acute aconi-tine poisoning.Methods Collected and analysed the data of 43 aconitine poisoning patients,summarize their clinic feature,ECG changes,laboratory test and treatment methords.Results All 43 cases showed arrhythmia under their ECGs,most of which were ventricular premature beats.Frequent multifocal ventricular arrhythmia,torsade de pointes,ventricular fibrillation occurred in severe aconitine poisoning patients.2 severe cases were treated with blood perfusion and purification in emergency and their arrhythmias were corrected rapidly.Serum K+ ,Mg2 + coucentrations decreased while myocardial enzymes increased in patients with severe and moderate poisoning.After active treatment,the ECG,cardiac creatase and electrolytes in all cases turned to normal.All 43 patients were cured.Conclusion The study shows the diversity of arrhythmia in acute aconitine poisoning patients and the deadly arrhyth-mia occurred in severe patients.Actively support therapy,early use of anticholinergic and antiarrhythmic drug,blood perfusion and purification in severe poisoning patients can rapidly cure arrhythmia and improve the rescue success rate.

ABSTRACT

Heat-treated and detoxified Aconite tuber has many useful properties such as analgesic, diuretic, and cardiotonic effects. Two hundred forty-seven patients were treated with Aconite Tuber powder (1.5 - 8.0g/day) added to Kampo extract granules. The Visual Analog Scale (VAS) was used for the assessment of its pain-relieving effect. One hundred two patients improved dramatically (the pre/post administration VAS ratio was less than 50%), 84 improved (between 51 to 75%), and61did not improved (more than 76%). Three patients(1.2%) showed adverse reactions, such as tongue numbness, squeezing sensation of the bladder, anasarca. These reactions were not serious. This study suggests that Aconite tuber powder can be used more widely, especially for arthralgia and somatic pain with coldness in elderly patients.

ABSTRACT

<i>Aconitum carmichaeli</i> debeaux is an essential herbal medicine that possess anti-inflammatory, analgesic, and cardiotonic effects. Five hundred ninety-three subjects were treated with Shuchi-bushi (a powder type of this aconiti tuber that has been autoclaved) added to Kampo extract granules, between the years 1996 and 2002. The patient population consisted of 153 men and 440 women with an average age of 54.5 years. Four patients showed adverse reactions such as hypertension, nausea and hot flashes with Shuchi-bushi. There were no serious adverse effects, and the toxic symptoms disappeared quickly by reducing the Shuchi-bushi dosage, or cessation.<br>Shuchi-bushi has been used in small quantities because of its highly toxic alkaloids. However, this study indicates that Shuchi-bushi can be used more actively.

ABSTRACT

A 43-year-old man who suffered from aconitine poisoning was treated effectively with Kampo medicine (Go-rei-san extracts). He showed polyuria and severe ventricular arrhythmia on the ECG. Ventricular arrhythmia improved in accordance with normalization of urination. Go-rei-san may be useful for the treatment of aconitine poisoning.

ABSTRACT

Both "caowu", the root of Aconitum kusnezoffii, "chuanwu", the main root of Aconitum carmichaeli are believed to possess anti-inflammatory, analgesics and cardiotonic effects. They have been used to treat conditions such as rheumatism, arthritis, bruises, fractures, hemiplegia. They contain the highly toxic C19 diterpenoid alkaloids of aconitine, mesaconitine and hypaconitine. After ingestion, patients may present with neurological, cardiovascular, gastrointestinal sign and symptoms. A case of thirty six-year-olds healthy women with aconitine poisoning due to caowu that is described presented as transient tetraplegia, arrhythmia and hypocalcemia.
